COVID-19 REPORTING QUESTIONNAIRE
APTS will be observing a holiday break from 8/30 until 9/6. During this time, if an APTS student or staff member experiences COVID-19 symptoms, are exposed, or test positive, please use the appropriate forms below to report the information to APTS nurses.
NOTE: If this is a medical emergency, please call 911 immediately or go to the nearest emergency room. This reporting system may not be monitored every day. You will only be contacted if follow-up is determined to be necessary by school nursing staff.
